Brady was sacked by Charles Woodson. He fumbled & the Raiders recovered but it was overturned. I don`t know if he threw the challenge flag? Maybe I`m just a bitter Rams fan? ;)

This is the one that the NFL is down on. They are pushing more for the City of Industry plan, they don't like the numbers on the Staples Center location.

The crown of helmet rule for runners will be impossible to referee though and makes little sense and so is NOT the solution. When you are running with the ball you want to get as low as possible, dipping your shoulders etc. In a crowd, sometimes you cant try and go round someone and have to go over them so you duck down and power into contact. You can lower your head if you're going through a pile. They don't want pointless 1 on 1 head contact in the open field.
